We ask you to read through the information below, and complete the steps 1-3. If you have any questions after reading through this information and instructions, please your supervisor/host.
Step 1: Submit the Self Declaration: Health, safety and environment
Self declaration HSE (online form)
Before you are granted access to the Department of Chemistry areas, this form has to be filled out and submitted. If you have not submitted the form prior to arrival, please do so on your first day. You are expected to familiarize yourself with the department's procedures for health, safety and environment topics (see links in the online form).
Step 2: Send us information about yourself
For employees, PhD students and guest researchers (> 6 month).
Information from you (online form)
In this online form we ask you to submit key information about yourself, including a short text about yourself/your background. We also ask you to upload a portrait photo of yourself. You can start on the form, save it, and continue later.
Based on the information you provide we will make an entry on our New Faces webpage, and establish your personal profile page at our People webpage.
